Water Survey

State regulations: every five years, we must do a cross connection survey and keep every single document on file for inspection. Our five years is up, and the village is sending water/sewer bills with this cross connection survey. The bill is stapled to the survey & cover letter.  1) This survey is coming in the mail so be watching for it. 2) It won’t take but just a few seconds to fill it out, and then return it to the office with your payment or in the drop box. Each survey must be signed and the address noted. 3) There is a penalty for failure to return the survey, and in bold print at the bottom of the document, it says “Water service may be disconnected for failure to complete and return the survey”.   4)  Rick’s phone and the office phone numbers may be used for contacts.  If anyone has questions, they can certainly call. 5) This is a state regulation and we’re doing what is necessary to stay in compliance.
